It known to mimic cancer due its similar clinical and radiological presentations. We underline a case 55-year-old male who presented with weight loss, jaundice, pruritus. Radiological imaging suggested mass, raising suspicion malignancy. However, subsequent evaluation, absence parenchymal lymphoplasmacytic cells on endoscopic ultrasound-guided biopsy, elevated serum immunoglobulin G4 level resulted in diagnosis AIP. Our emphasizes that AIP should be included differential obstructive especially when findings are inconclusive for cancer.
